Fractional CO₂ Laser
Fractional CO₂ laser is one of the most powerful and effective skin resurfacing treatments available in aesthetic dermatology. It delivers controlled ablative laser energy in a fractional pattern — treating thousands of microscopic columns of skin while leaving surrounding tissue intact. This triggers an intense wound-healing response that dramatically remodels collagen and elastin, achieving significant improvement in acne scars, skin texture, pore size, and signs of ageing.
How Fractional CO₂ Laser Works
The CO₂ laser (10,600 nm wavelength) is absorbed by water in skin cells, causing precise vaporisation of targeted tissue. In its fractional delivery mode, the laser creates thousands of microscopic treatment zones (MTZs) across the skin surface, each ablating a column of tissue down to a programmable depth. The surrounding untreated skin bridges accelerate healing, while the treated zones stimulate robust new collagen and elastin formation — resulting in smoother, tighter, and more even-textured skin.
Conditions Treated
Atrophic acne scars (boxcar, rolling, and shallow ice pick scars).
Surgical and traumatic scars.
Skin laxity and fine to moderate wrinkles.
Enlarged pores and rough skin texture.
Sun damage, mottled pigmentation, and uneven skin tone.
Stretch marks (striae distensae) in suitable cases.
The Treatment Process
1. Pre-Treatment Preparation
Pre-treatment skin priming with topical retinoids, depigmenting agents, or sunscreen is recommended for 4–6 weeks to optimise skin health and minimise the risk of post-laser pigmentation, particularly important for Indian skin types. Antiviral prophylaxis is prescribed for patients with a history of cold sores.
2. Treatment Session
A topical anaesthetic cream is applied 45–60 minutes before the procedure. In some cases, nerve blocks or additional analgesia may be used for deeper treatments. The laser handpiece is systematically passed over the treatment area with settings customised to the condition, scar depth, and skin type. The procedure typically takes 30–60 minutes depending on the area treated.
3. Immediate Post-Treatment
Significant redness, swelling, and a warm, sunburned sensation are expected immediately after treatment. A cooling mask or ice packs are applied. Patients are prescribed post-laser care including gentle cleansing, barrier creams, and strict sun protection.
4. Recovery Period
The skin typically peels and flakes over 5–7 days as the ablated columns shed and the surface re-epithelialises. Redness gradually subsides over 2–4 weeks. Patients with darker skin may experience temporary post-laser hyperpigmentation, which is managed with appropriate topical treatment.
Benefits of Fractional CO₂ Laser
Significant improvement in acne scars, skin texture, and tone.
Stimulates deep collagen remodelling for long-lasting results.
Fractional delivery balances effectiveness with manageable recovery.
Customisable depth and density for different skin concerns.
Combines multiple skin concerns in a single treatment session.
What to Expect
Most patients achieve 40–70% improvement in acne scars after 1–3 sessions, with collagen remodelling continuing for up to 6 months after each session. Deeper or more severe scars may benefit from additional sessions. A recovery period of 5–10 days of social downtime should be anticipated. Your dermatologist will design a treatment protocol based on your skin type, scar severity, and lifestyle considerations.
